Description:
This is an intermediate class for ages 12 - 17. Class meets from 4:30 - 5:30 pm, for 4 sessions, at Fish Park.
Our instructor is George Collier, USPTA/PTR

My participation in this program/activity is voluntary, and I have knowledge of the dangers involved and that my participation may result in injury. I hereby assume any and all risks of injury and release the City of Oakdale, the City of Oakdale Recreation Services Department, and their employees and/or agents of all liability and responsibility for injuries or damage that may occus as the result of my voluntary participation. I further acknowledge that I am familiar with the rules and regulations to be followed in this class/program/activity and agree to abide by those rules and regulations.
As a parent or guardian registering a minor child for a class/program/activity, I hereby accept the above stated Waiver and Release of Liability on behalf of said minor child.
